Leave of Absence Information
HR must be notified of any Leave of Absence of five days or more as soon possible.
Steps to reporting Leave of Absence of five consecutive days or more:
Step 1: Submit a Leave Affidavit in Talent Ed
Step 2: HR determines eligibility
Step 3: HR assigns FMLA checklist on Talent Ed.
Step 4: Employee submits FMLA application through Talent Ed and Medical Certification of Health Care Provider. Medical certification is not required for maternity leave.
Certificate of Health Care Provider for Employee's Serious Health Condition
Certificate of Health Care Provider for Family Member Serious Health Condition
Step 5: HR makes FMLA determination and notifies employee by writing.
Step 6: Employee notifies HR of return to work.
